Your First Visit

AT PERFORMANCE REHAB

What to Expect

Your first visit is considered the evaluation visit.
This visit usually takes a little longer than most other visits,
so please plan on being in the office for 60-90 minutes.

WHAT YOU

Need to Do

  • Please arrive 10 minutes early to complete paperwork.
  • Please bring your insurance cards and identification with you.
  • If you are on medication, please bring a current list of the medications and dosages.

For your first visit and all follow up visits, either wear or bring loose comfortable fitting clothing. Not only does the physical therapist need access to the injured area, you will also be moving around.

ADMINISTRATIVE

INSURANCE AND BILLING

Performance Rehab, Inc. is in network with most insurance companies. To verify that we are in network with your insurance, please call the office and ask for the billing department. In addition to private group health insurance, we will also bill Medicare, Medicaid, Worker’s Comp, and Motor Vehicle insurance.

Performance Rehab, Inc. bills all insurances every day.
PLEASE REMEMBER – YOU ARE ULTIMATELY RESPONSIBLE FOR YOUR PHYSICAL THERAPY COVERAGE.

If Performance Rehab, Inc. is not paid for your charges, you are responsible for payment. Depending on your insurance plan, you may be required to provide a co-payment to us at the time of service. Your plan may also have a deductible or a co-insurance.

If you do not have health insurance, we offer a reasonable out of pocket fee schedule. Please contact us for information regarding this.

Doctor’s Prescription vs. Referral

A referral is an authorization from your Primary Care Physician (PCP) referring you to a specialist. HMO insurance plans require a referral.

A prescription is a written order for physical therapy from the referring Doctor. Please check with your insurance carrier to find out if you need a PCP referral or if your physical therapy prescription will satisfy your insurance plan’s requirements.
